Program Expense Form
Fund advisors may request payment of a program expense from their fund by documenting the charitable purpose of the distribution. NOTE: Distributions from charitable funds must be made to a vendor; distributions to any "natural person" (individuals) are prohibited by law.
Fund advisors have two options for submitting a Program Expense Form:
Option 1: Complete the online form below, which requires that you upload an electronic copy of an invoice, statement, or receipt that reflects the expense.
Option 2: Download a PDF version of the form and send it with the required documentation (an invoice, statement or receipt that reflects the expense) to our office at 4745 Wheaton Drive, Fort Collins, CO 80525.
